Annual fees are set forth below:

1. A. Very small quantity generator: $100;
2. B. Small quantity generator: $500;
3. C. Large quantity generators that generate 100,000 pounds or less of hazardous waste annually: $5,000;
4. D. Large quantity generators that generate more than 100,000 pounds but less than 400,000 pounds of hazardous waste annually: $10,000;

5. E. Large quantity generators that generate 400,000 pounds or more of hazardous waste: $20,000.
