Each person having a radiation machine shall:

1. A. Apply for registration of such machine with the Department within 30 days following the effective date of these regulations or thereafter prior to the operation of a radiation machine on forms furnished by the Department and shall contain all the information required by the form and accompanying instructions; and

2. B. Designate on the application form an individual to be responsible for radiation protection.
