The information recorded shall be summarized and reported to the Department, on forms furnished by the Department, and shall be submitted within forty-five days after the end of the reporting period. Reporting periods are November 1 through April 30 and May 1 through October 31 or such other periods as the Department may deem necessary. Information reported to the Department shall be signed by the person responsible for its accuracy.
