A petition hearing shall be initiated by the filing of a variance petition or an appeal petition. The petitioner shall:

1. A. sign the petition under oath or affirmation and attest to the truth of the information contained therein; and
2. B. file the original and eight (8) copies of the petition with the board and serve a copy on the department.
