APPLICATION FOR PERMIT TO DRILL, DEEPEN OR PLUG BACK (Form C-101): Form C-101 is the form an operator uses to apply for a permit to drill, deepen, re-enter or plug a well back to a different pool or complete or re-complete a well in an additional pool.
