The notice shall contain, at a minimum, the following information:

1. A. the name and address of the applicant;
2. B. a description of the location and boundaries of the proposed permit area;
3. C. the purpose of the application and a general description of the mining operation;
4. D. the Director's name and address to whom written comments may be submitted and where a copy of the application is available for public inspection; and

5. E. the procedure and deadline for requesting a hearing.
