Written requests for a variance from any of the provisions of this rule shall:

1. A. state the reasons for the variance request;
2. B. identify each of the sections of this rule for which a variance is requested;
3. C. describe the effect the variance will have, if granted, on compliance with this rule;
4. D. describe how granting the variance will not compromise, or will further, the purposes of this rule; and
5. E. indicate why the proposed variance is a reasonable alternative to the requirements of this rule.
