Any entity holding a certificate of public convenience and necessity or a certificate of operating authority for the provision of local exchange services shall immediately notify the commission in writing of any change(s) in circumstances, including, but not limited to, any of the following: (1) a change in the entity's name; (2) a merger of the entity with another entity; (3) acquisition of the entity by another entity; (4) acquisition by the entity of another entity; and (5) transfer of a significant portion of the entity's assets to another entity.
