   2. (2) Initial license: Pro-rated at $5.00 per month of initial license term.
   3. (3) Temporary license: $25.00.
   4. (4) Expedited license: Pro-rated at 5.00 per month of initial license term.
   5. (5) Biennial renewal: $125.00.
   6. (6) Late renewal penalty: $75.00.
   7. (7) Renewal during the grace period: $125.00 plus late renewal penalty fee.
   8. (8) Reactivation from inactive status: $125.00 plus late renewal penalty fee.
   9. (9) Review fee for “other” elective courses as provided in 16.7.4.14 NMAC: $50.00.

2. B. Massage therapy school:

   1. (1) Registration: $50.00.
   2. (2) Annual renewal: $50.00.
   3. (3) Late renewal penalty: $75.00.
   4. (4) Application review: $400.00.
   5. (5) Curriculum change review: $50.00.

3. C. Administrative fees:

   1. (1) Paper lists: $50.00.
   2. (2) Labels: $75.00.
   3. (3) Electronic list: $125.00.
   4. (4) Replacement license: $25.00.
   5. (5) Verification of license $15.00.
   6. (6) Other administrative fees (at the discretion of the board or board administrator) not to exceed $500.00.

4. D. Continuing education fees:

   1. (1) One-time fee for course taught by a continuing education provider who was an active New Mexico registered independent massage therapy instructor on February 4, 2019: $50.00.
   2. (2) One-time application fee for a course taught by an individual or other entity who is not a continuing education provider, per each new course: $50.00.

5. E. All fees collected by the board are non-refundable.
