An application for licensure as a speech-language pathologist by examination must be accompanied by the following documents:

1. A. official transcripts verifying at least a master's degree in speech-language pathology, speech-language and hearing science, communication disorders or equivalent degree regardless of degree name; or
2. B. a certification bearing an official seal and attesting to completion of degree requirements from the registrar, mailed directly to the board from the conferring institution; and
3. C. proof of having earned a certificate in speech-language pathology from a board recognized national speech-language association or proof of completion of the clinical fellowship year or equivalent; and
4. D. proof of having passed a nationally recognized standard examination in speech-language pathology;
5. E. proof of having passed the jurisprudence examination with a grade of no less than seventy percent.
