All fees are payable to the board and are non-refundable. No individual fee shall exceed $150.00 annually. Fees are as follows.

4. D. Application for euthanasia instructor certification $ 150.00
5. E. Renewal for euthanasia instructor certification $ 150.00
6. F. Application for euthanasia agency license $ 150.00
7. G. Renewal for euthanasia agency license $ 150.00
8. H. Late renewal fee (includes technician, instructor and agency) $ 125.00
9. I. Verification of licensure or certification $ 30.00
10. J. Listing of licensees (paper or electronic) $ 150.00
11. K. Charge for insufficient funds $ 65.00

12. L. Duplicate licenses $ 25.00
