N.M. Code R. § 16.23.1.15

Duplicate License

Effective Jun 27, 2023State of New Mexico

In the event a license or permit is lost or destroyed, the department will issue a duplicate license or permit upon receipt of the following.

A. Notice to the department of the loss by the licensee or permitee.
B. A request for a duplicate.
C. Any applicable fee.
