If a licensee or permit holder requests a new license or permit, as the result of a legal name change, the department will issue a new license upon receipt of the following:

1. A. the old license(s) or permit(s);
2. B. legal proof of the name change;
3. C. a written request for name changes to be made on licensing or permit records; and
4. D. any applicable fee.
