1. A. All fees shall be paid by check, certified check or money order in US funds unless otherwise specified by rule.

   3. (3) application for licensure by endorsement: $525.00;
   4. (4) application for temporary licensure: $330.00;
   5. (5) application for limited temporary license: $100.00;

4. (6) clinical skills examination, not including the cost of any nationally recognized examinations: $500.00;

   1. (7) annual license renewal: $225.00;
   2. (8) late license renewal: an additional $200.00;
   3. (9) expired license renewal: an additional $350.00 plus the renewal and late fees;
   4. (10) temporary license renewal: $100.00;
   5. (11) application for a new annual approval or renewal of approval of an educational program, including the same program offered at multiple campuses: $450.00;
   6. (12) late renewal of approval of an educational program: an additional $200.00;
   7. (13) application for single instance approval of an educational program: $225.00;
   8. (14) application for initial expanded practice certification: $100.00 per module;
   9. (15) application for triennial expanded practice license renewal: an additional $200;
   10. (16) late expanded practice license renewal: an additional $125.00 plus the renewal fee;
   11. (17) expired expanded practice license renewal: an additional $100.00 plus the renewal and late fees;
   12. (18) application for externship supervisor registration: $225.00;
   13. (19) application for extern certification: $225.00;
   14. (20) continuing education provider course approval application: $50.00;
   15. (21) auricular detoxification specialist certification application: $50.00;
   16. (22) auricular detoxification specialist certification renewal: $30.00;

   6. (34) administrative fee for inactive license reinstatement application: $125.00;
   7. (35) administrative fee for each duplicate license: $30.00;

6. (36) administrative fee for a single transcript or diploma from the former international institute of Chinese medicine, per copy: $50.00;

   1. (37) administrative fees to cover the cost of photocopying, electronic data, lists and labels produced at the board office.
