1. A. Letters of reference, if applicable.
2. B. Medical reports and/or records of chemical dependency, physical or mental examinations or treatment in an impaired practitioner program.
3. C. Examination scores.
4. D. The contents of any examination used to test for an individual’s knowledge or competence.
5. E. Investigative files.
6. F. Written and oral communication relating to actual or potential disciplinary action, including complaints.

7. G. Matters of opinion.
