1. A. A physician assistant may request a change from supervised to collaborative status by submitting verification of at least three years of clinical practice supervised by a licensed physician and provide proof of current malpractice liability insurance. This change will allow the physician assistant to work as a collaborative or supervised physician assistant.
2. B. A physician assistant may request a change from collaborative to supervised status by submitting a supervising physician form to the board.
