(1) the full name, address, and license number of the person receiving the gaming device;

1. (2) the full name, address, and license number of the person from whom the gaming device was received;
2. (3) the date of receipt of the gaming device;
3. (4) the serial number of each gaming device;
4. (5) the model number and description of each gaming device;
5. (6) the manufacturer of the gaming device;
6. (7) the location where the gaming device will be placed and the license number of the licensed premises;
7. (8) the expected date and time of installation of the gaming device at the new location; and
8. (9) such other information as required by the board.

C. Prior to transporting a stored gaming machine from one location to another location within the state, the licensee shipping the gaming machine shall comply with the requirements of 15.1.16.11 NMAC relating to intra-state transportation of gaming machines.
