A. The record of the proceeding shall include:

1. (1) all pleadings, motions, and intermediate rulings;
2. (2) evidence received or considered;
3. (3) a statement of matters officially noticed;
4. (4) questions and offers of proof, objections and rulings thereon;
5. (5) proposed findings and conclusions; and
6. (6) any action recommended by the hearing examiner.

B. A party may request a transcription of the proceedings. The party requesting the transcript shall bear the cost of transcription.
