Each self-insured shall prepare an annual report on the form provided by the superintendent. The report shall be filed within ninety (90) days after the end of the self-insured’s fiscal year and shall be accompanied by the following documents:

1. A. a certified annual financial statement;
2. B. actuarial reports, unless waived by the superintendent, attesting to the loss reserve requirements and projected losses and loss adjustments expense for the coming year;
3. C. a report of automobile claims activity;
4. D. a copy of its current plan of operations; and

5. E. a copy of the current declarations page and all endorsements providing or limiting excess insurance coverage in New Mexico.
