The first page of the endorsement must contain:

1. A. Space for insertion of name of company or companies issuing the endorsement and other matter permitted to be stated at the head of this endorsement.

2. B. Space for description of insured motor vehicle or automobile, listing of rates and premiums for the basic limits coverage insured under the endorsement and for additional excess limits of coverage under the endorsement.
