This rule applies to all health insurance carriers, including health maintenance organizations, individual health plans, group and blanket health plans, provider service networks and nonprofit healthcare plans that offer or administer health benefits plans, including health benefits plans and managed health care plans subject to the insurance laws and regulations of this state.
