1. A. Appendix B due date. On or before March 1 of each year, an issuer shall report to the superintendent the following information for every individual resident of this state for which the issuer has in force more than one Medicare Supplement policy or certificate using the form referenced in Subsection B of 13.10.26.31 NMAC:

   1. (1) policy and certificate number; and
   2. (2) date of issuance.
2. B. Report organization. The items set forth in Subsection A of this section must be grouped by individual policyholder.
