B. The form shall require of the requester:

1. (1) printed name and signature of the requester;
2. (2) intended use of materials by the requester; and

3. (3) collection information, which may include the following:

   1. (a) collection title;
   2. (b) accession number
   3. (c) box and folder number; and
   4. (d) description of item.

C. The form shall require of the archivist:

1. (1) serial number of container and vault location for each item;
2. (2) total number of items retrieved;
3. (3) signature of archivist retrieving items and date and time; and

4. (4) signature of archivist re-shelving items and date and time.
