N.H. Code Admin. R. We 801.01 – Required Information and Entry Format

(a) “Well Completion Reports” shall include the following information:
1. (1) Property owner name;
2. (2) Building contractor name, if applicable;
3. (3) Location of well, located by:
  1. a. Geographic coordinates provided by global positioning technology;
  2. b. Enhanced 911 street address, if available;
  3. c. Tax map and lot number of the property; and
  4. d. A hand drawing or site map, if more than one well is located on the property;
4. (4) A statement identifying whether the well site requires a setback reduction form;
5. (5) Date well was completed;
6. (6) Proposed use of well;
7. (7) Reason for constructing well;
8. (8) Type of well;
9. (9) Total depth of well;
10. (10) Depth to bedrock, if encountered;
11. (11) Casing details;
12. (12) Method of sealing casing to bedrock, if applicable;
13. (13) Screen details, if applicable;
14. (14) Results, method, and duration of yield test(s) conducted;
15. (15) Static water level, if encountered;
16. (16) A statement specifying whether a water quality sample was collected;
17. (17) A description and depth intervals of unconsolidated surficial deposits encountered;
18. (18) Description of competent bedrock type, if encountered;
19. (19) Yield test at various depths, if performed;
20. (20) Hydro-fracturing or other well development details, if applicable;
21. (21) Additional well seals installed, if applicable;
22. (22) Special notes, if any;
23. (23) Company name;
24. (24) Date of report; and
25. (25) License number.
(b) “Well Completion Report” entries shall be typed or printed.
(c) “Well Completion Reports” shall be signed by the licensed water well contractor.
