Each applicant for licensure shall complete and submit the “Addendum to the Universal Application for Initial License for Veterinarians” revised 7/2025 by attesting to the following:
- (a) The applicant is 18 years of age or more;
- (b) The applicant is either a graduate of an AVMA accredited school of veterinary medicine or the holder of an ECFVG certificate, a PAVE certificate, or certificate through any other certification program that meets the ECFVG or the PAVE program minimum standards;
(c) If the applicant is not a graduate of an AVMA accredited school of veterinary medicine, the applicant has requested through the American Association of Veterinary State Board’s Veterinary Application for Uniform Licensure Transfer (VAULT) portal the following:
- (1) A letter from the AVMA verifying that the Education Commission for Foreign Veterinary Graduates (ECFVG) certification program was completed; or
- (2) A certified copy of the Certificate of Qualification issued by the Canadian Veterinary Medical Association;
- (d) The applicant has requested certified transcripts from their veterinary school to be submitted directly to the OPLC. If transcripts are not in English, an officially translated copy shall be submitted with the transcripts;
- (e) The applicant has requested their exam scores to be submitted to the OPLC directly from Veterinary Information Verifying Agency (VIVA) to demonstrate that the applicant has passed both the National Board Exam (NBE) and the Clinical Competency Test (CCT), or the North American Veterinary Licensing Exam (NAVLE); and
- (f) If the applicant is applying through this state to take the NAVLE, the applicant has submitted the NAVLE application and the NAVLE online approval form directly to the NBVME by the deadlines posted on that website.
