The witnessed signature on page 9 of the application shall have the effect of certifying that:

1. (a) Any description placed by the applicant on page 9 of his or her non-annulled conviction of a crime is accurate; and
2. (b) The failure of the applicant to place such a description on page 9 signifies that the applicant has no non-annulled conviction of a crime.
