(a) An individuals certified as a speech-language assistant shall send to the board’s office within 30 days any changes in:

1. (1) His or her name;
2. (2) Home or place of employment physical addresses;
3. (3) Home or place of employment mailing addresses;
4. (4) Home or place of employment telephone number or personal cell phone number if provided to the board.
5. (5) E-mail address;

(b) Licensees shall report to the board’s office within 30 days:

1. (1) Any misdemeanor or felony conviction;
2. (2) Any act which constitutes misconduct under Spe 404.01; and
3. (3) Any license or other proof of licensure that was issued by the board and has been lost or stolen.
4. (b) Individuals certified as speech-language assistants shall retain documentation of their continuing professional education and professional activities for at least 3 years.

5. (c) Individuals holding a currently valid certificate as a speech-language assistant under these rules shall use one of the following after his or her written name:

   1. (1) Speech-Language Assistant;
   2. (2) Certified speech-language assistant;
   3. (3) “SLA”; or
   4. (4) “CSLA”.
6. (d) Unless certified under these rules, no person shall represent himself or herself, or the services offered by using the letters “SLA” or “CSLA” or the words “speech-language assistant”, “certified speech-language assistant”, or any other similar words, if the intent of such use is to imply that the person is certified.

7. (e) Certified speech-language assistants shall retain documentation of their continuing professional education and professional activities for at least 3 years.
