An individual applying for reinstatement of certification after his or her certification has lapsed for longer than 2 years shall submit to the board:

1. (a) A completed, dated, signed and notarized reinstatement application form;
2. (b) The supporting materials specified in Spe 606.06; and
3. (c) Payment of the reinstatement fee set forth in Ahp 301.02(a).
