Any individual or organizational entity seeking approval from the board to provide pre-licensing education and fieldwork programs shall submit:

1. (a) A “Septic System Evaluator Training Program Application”, effective 2/2019; and

2. (b) A general outline of the education and fieldwork programs, including but not limited to:

   1. (1) The total number of hours;
   2. (2) The expected course outcomes;
   3. (3) Syllabus;
   4. (4) Materials;
   5. (5) Minimum requirements of Sep. 603 and Sep.701.02;
   6. (6) Information and basic knowledge on proprietary systems; and

   7. (7) Credentials of the instructor(s).
