Each applicant for licensure shall provide the following information to the licensing bureau on the “Addendum to the Universal Application for Initial License for Septic System Evaluators”, revised August 2025:

1. (a) A “yes” or “no” answer to the question “Does the applicant hold a Granite State septic system certified evaluator designation pursuant to RSA 310-A:210, II?”;

2. (b) A “yes” or “no” answer to the questions “Has the applicant completed the classroom and field training from a board approved septic evaluator provider, pursuant to RSA 310-A:210, I(a)? If “yes,” has the applicant provided documentation issued by the course provider that includes the following information:

   1. (1) The date of the course;
   2. (2) Where the course was taken; and
   3. (3) Name of the course provider;
3. (c) A “yes” or “no” answer to the question “Is the applicant at least 18 years old as required by RSA 310-A:210, I(c)?”.
4. (d) A “yes” or “no” answer to the question “Has the applicant passed a board approved examination required by Sep 303.01?”.
