Business applicants for licensure shall provide the following documents supporting their applications:
- (a) A photocopy of the New Hampshire master plumber and/or fuel gas fitting license held by the person who signed the application;
- (b) A letter signed by the managing members, corporate officers, partners, and/or business entity owner stating that the person who signed the application is the responsible managing employee or owner of record for the business entity; and
- (c) If the applicant is a business that is incorporated, formed or located outside of New Hampshire, a photocopy of its certificate of authority to conduct business in New Hampshire.
