1. (a) Each request for a hearing shall be in writing and contain the following information:

   1. (1) Name, address, and telephone number of person requesting the hearing;
   2. (2) Make and registration of vehicle in question;
   3. (3) Date and time vehicle was removed from its location;
   4. (4) Location of vehicle after removal;
   5. (5) Name of authorized officer involved, if known; and
   6. (6) Reason for contesting the removal.

2. (b) All requests for hearings shall be addressed to:
