1. (a) The renewal process may be initiated by the driver-applicant 60 days prior to the expiration date of the waiver.
2. (b) All persons seeking a renewed medical waiver shall submit DSMV 558, “Application for Waiver of Physical Deficiency”, (rev. 06/23).
3. (c) Each driver-applicant who has a medical waiver because they are not physically qualified pursuant to Saf-C 909.07(c), shall be required to obtain a current medical evaluation summary in order to renew a medical waiver.
