1. (a) Each applicant for a waiver of the physical disqualification shall submit an application to the commissioner. The application shall be addressed and mailed to:

   Department of Safety

   Division of Motor Vehicles

   23 Hazen Drive

   Concord, NH 03305

   Attn: Medical Waiver Program

2. (b) The application in (a) above shall be submitted jointly by the driver-applicant requesting the waiver and by the motor carrier that will employ the driver-applicant if the waiver is granted, who shall be the co-applicant.
3. (c) A driver-applicant who is self-employed or unemployed shall not be required to have a co-applicant.
