(a) Any person applying for permission to connect an alarm shall furnish to the receiving agency the following:

1. (1) The name, address, and telephone number of the subscriber;
2. (2) The type of protection afforded by the alarm, whether burglary or fire;
3. (3) The name, address, and telephone number of 2 persons to be called in the event of alarm activation;
4. (4) Whether the alarm is monitored by any other person or agency, and if so, the name, address, and telephone number of same;
5. (5) The name, address, and telephone number of the alarm service company;
6. (6) The subscriber’s signature; and
7. (7) The approving officer's signature.

(b) The information above shall be provided on a form prescribed by the receiving agency. The department shall make available standardized application forms which the receiving agency may utilize.
