- (a) An applicant shall complete and submit DSFS 92, “Application for Proximate Audience License” (rev. 12/21).
- (b) The applicant shall sign and date form DSFS 92 under the penalty of unsworn falsification, pursuant to RSA 641:3, and in doing so, shall certify the following: “I certify that I have read the foregoing and affirm that every statement contained herein is true and correct. I also certify that I am familiar with all state laws, regulations and local ordinances relating to the use and handling of these materials for the location in which I intend to conduct displays of special effects, pyrotechnic effects, or flame effects.”
- (c) The applicant shall complete and submit a DSSP 256 form “Criminal Record Release Authorization Form” to authorize the department to obtain the applicant’s criminal record.
- (d) An incomplete application shall be returned to the applicant, along with an explanation. The applicant may resubmit.
