1. (a) A permit fee of $250 shall be charged by the department for the following:

   1. (1) Any display utilizing only pyrotechnic effects on a single stage; or
   2. (2) Any display utilizing only flame effects on a single stage.

2. (b) A permit fee of $400 shall be charged by the department for the following:

   1. (1) Any display utilizing both pyrotechnic effects and flame effects;
   2. (2) Any display utilizing pyrotechnic effects, flame effects, or both, on more than one stage;
   3. (3) Any display utilizing pyrotechnic effects, flame effects, or both, on more than one level of the building separate from the stage;
   4. (4) Any display utilizing pyrotechnic effects, flame effects, or both, on any motorized vehicle or motorized stage equipment; or
   5. (5) Any application consisting of 10 or more performances or events.
3. (c) The applicant shall pay the permit fee prior to the issuance of the permit.
