Monitoring reports shall include at minimum:

1. (a) A statement that the device:

   1. (1) Is calibrated;
   2. (2) Has been inspected and is functioning properly; and
   3. (3) Shows no evidence of tampering; and
2. (b) The, date, time, and location of the next monitoring check;
3. (c) The name, location, and telephone number of the interlock service provider;
4. (d) Photos supporting each breath test or attempt within the report;
5. (e) The date and time of each event listed;
6. (f) The name, date of birth, and address of the user;
7. (g) Driver license number and driver license state of the user;

8. (h) The following vehicle information:

   1. (1) Make, model, and year;
   2. (2) Vehicle identification number;
   3. (3) License plate number and state of issuance; and
   4. (4) Odometer reading; and
9. (i) Calibration readings of the device;
10. (j) Make, model, and serial number of the device;
11. (k) Dates of the monitoring period;
12. (l) Any comments from the interlock service provider; and
13. (m) Jurisdiction(s) where the user is being monitored.
