Violation reports shall include at a minimum:

1. (a) Photos supporting each breath test or attempt within the report;
2. (b) The data captured from at least 5 breath attempts leading up to any violation and the data captured from the subsequent 5 events;
3. (c) The date and time of each event listed;
4. (d) The name, date of birth, and address of the user;
5. (e) Driver license number and driver license state of the user;
6. (f) The name, address, and telephone number of the interlock service provider;

7. (g) The following vehicle information:

   (1 Make, model, and year;

8. (2) Vehicle identification number; and
9. (3) License plate number and state of issuance; and
10. (h) Make, model and serial number of the device;
11. (i) Dates of the monitoring period;
12. (j) The name, location, and telephone number of the interlock service provider who performed the calibration or event that identified the violation;
13. (k) Any comments from the interlock service provider; and
14. (l) Jurisdiction(s) for which the user is being monitored.
