1. (a) Upon receipt for each sample received pursuant to RSA 265-A:4, the following information will be recorded:

   1. (1) A forensic laboratory number will be assigned to the sample; and

   2. (2) Record evidence custody information, including:

      1. a. The date, time, and purpose of transfer;
      2. b. The name of the person relinquishing the sample; and
      3. c. The name of the person receiving the sample.
2. (b) Any problem encountered with the sample from the time of receipt to the date of destruction shall be noted in the case file.
3. (c) A sample submitted in accordance with Saf-C 6402.07 shall be placed in a secure refrigerator until the time of testing or destruction.
4. (d) Forensic laboratory personnel shall maintain a record of evidence custody to document all handling of the sample until the sample is released or destroyed.
