Captured Sample Analysis Report

(a) The report of the analysis of a captured sample performed by an independent laboratory pursuant to RSA 265-A:7 shall include:
- (1) The serial number of the captured sample bag;
- (2) The serial number of the approved instrument from which the breath sample was captured;
- (3) The subject’s reported value identified on the printout from the approved instrument in accordance with Saf-C 6302.04;
- (4) The results of the captured sample analyzed in duplicate in accordance with Saf-C 6305.02(a)(5) and (6);
- (5) The independent laboratory’s reported value as determined in accordance with Saf-C 6305.03(a); and
- (6) The name of the person conducting the analysis.
