1. (a) Any person requesting a variance or exception pursuant to this part shall submit a completed, dated, and signed form DSFS 129, “Request for Variances or Exceptions Pursuant to NH Code of Administrative Rules Saf-C 6005.04” (rev. 5/20) and submitting it to:

   Department of Safety

   Division of Fire Safety

   James H. Hayes Building

   33 Hazen Drive

   Concord, NH 03305

2. (b) The applicant shall forward the local fire department a copy of form DSFS 129.
