1. (a) An application for a unit, provider, PEETE, educational training agency, EMS I, EMS I/C, chair van company, or wheelchair van for hire license shall be reviewed by the division in accordance with RSA 541-A:29.
2. (b) The applicant shall be notified of any apparent errors or omissions in the application and allowed to resubmit the application within 30 days of initial receipt by the division.

3. (c) Applications for a unit, provider, PEETE, educational training agency, EMS I, EMS I/C, chair van company, or wheelchair van for hire license shall be denied in accordance with RSA 541-A:29 for:

   1. (1) Failure to meet the qualifications;
   2. (2) Failure to submit required documentation;
   3. (3) Failure to complete a suspension period, pursuant to Saf-C 5922.04(e), or revocation period pursuant to Saf-C 5922.05(d);
   4. (4) Falsifications or omissions of items from a criminal background check; or

   5. (5) A conviction of any criminal offense:

      1. a. Relating to the performance of duties or practice of EMS; or
      2. b. That endangers the health or safety of the public.
4. (d) Any applicant issued a provisional license in accordance with Saf-C 5904.05(d) shall have said licensed revoked upon the receipt of a disqualifying criminal history record check pursuant to Saf-C 5922.03 (c)(5).
5. (e) An applicant aggrieved by the decision of the commissioner relating to a denial of an application may request an adjudicative hearing in accordance with RSA 541-A and Saf-C 200.
