1. (a) All wheelchair van companies shall be responsible for recordkeeping and reporting.
2. (b) All reports submitted to the division shall include the company license number.

3. (c) All companies shall report the following, in writing, to the division within 30 calendar days:

   1. (1) All new operators affiliated with the company, which shall include the operator’s legal name;
   2. (2) All operators who are no longer affiliated with the company, which shall include the operator’s legal name;

   3. (3) Relocation of the company, which shall include both the old and the new company’s:

      1. a. Street and mailing address;
      2. b. City or town;
      3. c. State;
      4. d. Zip code;
      5. e. Telephone number;
      6. f. Primary contact person’s name; and
      7. g. A preferred E-mail address;
   4. (4) Additions and deletions of wheelchair vans licensed to the company; and
   5. (5) Changes to the company ownership and leadership.
