Each applicant for a wheelchair vans for hire license application shall complete and submit “New Hampshire Bureau of EMS NH Wheelchair Van for Hire Van License Application”, form, as revised 7/18/76, available at https://www.nh.gov/safety/divisions/fstems/ems/documents/wheelvan.pdf.
