EMS I AND EMS I/C License Application

Each applicant for an EMS I or EMS I/C license shall complete and submit “New Hampshire Bureau of EMS EMS Instructor/Coordinator License Application” form, as revised 4/2024, available at https://www.nh.gov/safety/divisions/fstems/ems/documents/a29.pdf.
