EMS Vehicle License Application

Each applicant for an EMS vehicle license shall complete and submit “New Hampshire Bureau of EMS Transporting Vehicle Application” form, as revised 01/07/19, available at https://www.nh.gov/safety/divisions/fstems/ems/documents/formvehicle.pdf.
