Person Requesting their Own Record

- (a) A person who is requesting his/her own information shall complete form DSMV 505, by providing his or her name, address, date of birth, and specifying what information s/he wants to receive by checking the appropriate box.
- (b) The person requesting his or her own motor vehicle record shall execute DSMV 505 under penalties of unsworn falsification.
- (c) In addition to a properly completed DSMV 505, the person shall provide proof of identity if requesting information in person.
