1. (a) Each interested person applying to register for a motorcycle rider education program course shall complete and submit DSMV 414 “Motorcycle Training Course Registration Form” (Rev. 12/24), available either:

   1. (1) Online at https://www.dmv.nh.gov/sites/g/files/ehbemt416/files/inline-documents/

      dsmv414.pdf; or

   2. (2) By mail or in person at

      Division of Motor Vehicles

      ATTN: Motorcycle Rider Training Program

      23 Hazen Drive

      Concord, NH 03305

2. (b) By signing the form, the applicant shall certify that the application is signed under the penalty of unsworn falsification pursuant to RSA 641:3.
