Certification by a Physician, Physician Assistant, Podiatrist or Advanced Practice Registered Nurse

(a) A licensed physician, physician assistant, podiatrist, or advanced practice registered nurse shall provide the following information regarding an application for a walking disability plate or placard:
- (1) The medical condition, as specified in RSA 259:124 and 261:88, that limits or impairs the applicant’s ability to walk;
- (2) How the medical condition limits or impairs the applicant's ability to walk; and
- (3) Whether the medical condition that limits or impairs the applicant’s ability to walk is likely to improve.
- (b) For the purposes of RSA 261:88, I(c) and this section, "other medically debilitating conditions" means a diagnosed medical condition, as determined by a licensed physician, physician assistant, podiatrist, or advanced practice registered nurse, that limits or impairs a person's ability to walk in such a manner that the applicant's health would be adversely affected without a walking disability plate or placard.
- (c) The licensed physician, physician assistant, podiatrist, or advanced practice registered nurse shall sign and date the form under penalty of unsworn falsification. A stamped signature from a licensed physician, physician assistant, podiatrist, or advanced practice registered nurse shall not be accepted.
